Job summary

WellBe Senior Medical is hiring a Nurse Practitioner to deliver field-based primary care in patients’ homes across Rochester, NY. The role emphasizes autonomy, a holistic care approach, and collaboration with an interdisciplinary team to improve outcomes.

Compensation includes a base salary in the six-figure range with mileage reimbursement, full benefits, and relocation assistance. This independent, in-home model supports seniors with comprehensive care outside traditional clinics.

Qualifications

• Master’s Degree in Nursing (NP) with focus on Adult, Geriatric, Acute Care and/or Family.
• Active NP license and board certification (ANCC or AANP).
• Valid driver’s license and vehicle for local field-based travel.

Responsibilities

• Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the patient’s home.
• Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
• Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
• Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.

Education
