Job summary

Concierge Home Care in Marianna, FL is seeking an experienced Licensed Clinical Social Worker (LCSW) for home health, offering flexible scheduling and patient-centered care in patients' homes.

You will provide psychosocial support and counseling, develop care plans, and connect families with community resources while collaborating with healthcare teams. AHCA Level 2 background screening may be required.

Qualifications

• Active Florida Licensed Clinical Social Worker (LCSW) license.
• Minimum of two years of clinical counseling experience.
• Home health or telehealth experience preferred.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Provide psychosocial assessments, counseling, and behavioral health support.
• Develop and coordinate care plans addressing social, emotional, and environmental needs.
• Connect patients with community resources and support services.
• Assist patients and families in navigating social, financial, and healthcare challenges.
• Collaborate with physicians and interdisciplinary care teams to support patient outcomes.
• Educate patients and caregivers on coping strategies and available resources.
• Maintain accurate and timely documentation in accordance with Medicare guidelines.
• Document visits efficiently using AI-assisted EMR tools.
