Job summary

Family-Outreach,-Inc. seeks a dedicated Family Support Specialist to empower families raising children with developmental disabilities or delays.

You will perform assessments, plan individualized care, and coordinate services with a multidisciplinary team in a home-based setting. Training in Routine Based Interviews (RBI) will be provided, and you will support families through coaching, education, and referrals, while ensuring HIPAA compliant documentation and professional development.

Qualifications

• Bachelor’s Degree from an accredited college or university.
• Ability to complete FSS certification process within Montana Milestones timelines.
• Valid Montana driver’s license or ability to obtain one within one month of hire.
• Access to a reliable vehicle.
• Successfully complete a background check.

Responsibilities

• Conduct assessments of family and child needs to determine eligibility and develop an action plan.
• Becoming trained or certified in Routine Based Interviews (RBI).
• Develop individualized care plans based on multidisciplinary input.
• Coordinate services with other agencies for a multidisciplinary team approach.
• Maintain accurate tracking and thorough documentation of services in state-compliant systems.
• Explain policies and procedures to families and inform them of legal rights.
• Coach best practices in early childhood behavior management to families during monthly home visits.
• Monitor child outcomes related to social-emotional development and learning goals.
