Job summary

Jewish Family & Children's Service of Arizona is seeking a Family Connections Consultant II to provide homebased and community services to families referred by the Department of Child Safety (DCS) across Maricopa and Pinal Counties. You will identify needs, plan services, and coordinate care with DCS and other agencies.

The role requires a Bachelor’s degree in social services, at least one year in social services, and a valid AZ driver's license.

Qualifications

• Bachelor's degree in social service field required.
• One year of social service work experience required.
• One year experience as Family Consultant I preferred.
• Familiarity with the child welfare system strongly preferred.
• Valid Arizona Driver's License and reliable transportation required.

Responsibilities

• Provide homebased and community services in Maricopa and Pinal Counties to families referred by DCS.
• Perform outreach, assessment, service planning, and coordinated care with DCS and other services.
• Conduct weekly home visits and monitor child safety, reporting suspected abuse.
• Support positive parenting and refer to appropriate programs when needed.
• Ensure documentation in EMR within 48 hours of contact.
• Assist in onboarding/training of new hires.
