Job summary
Acentra Health is looking for a Clinical Assessor to join our growing team in Morven, North Carolina. This hybrid position requires candidates to perform assessments for Medicaid beneficiaries in their homes or adult care facilities, providing vital services through the Personal Care Services program.

Qualifications
- Registered Nurse or LCSW license by the applicable state.
- Minimum of two years of nursing experience.
- Ability to travel up to a 60 mile radius.
Responsibilities
- Conduct needs-based eligibility determinations for Medicaid beneficiaries.
- Complete needs-based assessments of level of care.
- Provide community-based services to prevent institutionalization.
