AxessPointe Community Health Center, Inc. is seeking a Nurse Practitioner to provide primary medical and behavioral health services to adults, children and families in northeast Ohio under a collaborative agreement and API Medical Director guidance.

The NP/PA will examine, diagnose, treat, order tests, prescribe medications, and promote preventive care while collaborating with a care team to deliver patient-centered services across locations.

Qualifications

• Formal MSN-NP/PA training and certification in the state of Ohio to practice medicine.
• Knowledge of preventative medicine principles and practices.
• Understanding of state and federal medicine/public health laws.
• Experience collaborating with physicians and care teams.

Responsabilités

• Examine, diagnose, and treat illnesses and injuries across the lifespan.
• Provide well-child, well-woman, and well-man exams for patients.
• Order appropriate labs and imaging; interpret results for care plans.
• Prescribe medicines and treatment modalities per authority.
• Promote health education, disease prevention, and patient empowerment.
• Document care and collaborate with specialists as needed.
