Job Summary

This position is responsible for performing a variety of functions and is a single source of contact for members, health care personnel, and all other entities involved in managing care. The role involves identifying alternative treatment programs, consulting with physicians, providers, members, and other resources to evaluate options and services needed to meet an individual's health needs. The incumbent promotes quality and cost-effective outcomes and serves as a liaison to physicians and members. Additional functions include determining member assignment of treatment, pre-admission and post-discharge counseling for acute conditions, establishing relationships with members, handling immediate post-discharge follow-up, providing education and local resource information, and encouraging member self-education. The role also includes assessing members for wellness education and disease management, introducing members to website tools, educating on wellness and specific conditions, and facilitating care coordination for identified members. Oversight of holistic health coordinators is also part of the responsibilities.

Job Requirements
  • Registered Nurse (RN) with current, valid, unrestricted license in the state of operation
  • Compact license
  • At least 2 years of clinical practice experience providing direct clinical care to customers
  • At least 1 year of experience in care or condition management within a health insurance or managed care setting
  • Proficiency in PC applications including Word, Excel, Lotus Notes, and database management
  • Clear and concise verbal and written communication skills
  • Familiarity with ancillary services such as HHC, SNF, Hospice, etc.
  • Analytical skills
  • Decision-making skills and sound clinical judgment to oversee staff
Preferred Job Requirements
  • Experience managing complex or catastrophic health cases
  • Specialty clinical experience in areas such as intensive care, orthopedics, NICU/pediatrics, oncology, diabetic management, or obstetrics
  • Progress toward or completion of CCM/CCP/CDE/MCG certification or an advanced degree
  • Knowledge of Milliman Guidelines or similar clinical guidelines
  • Knowledge of medical management policies and procedures

Base Pay Range

$60,300.00 - $133,400.00
