This position is responsible for performing a variety of functions and serving as the single source of contact for members, healthcare personnel, and all other entities involved in managing care. The role involves identifying alternative treatment programs, consulting with physicians, providers, members, and other resources to evaluate options and services to meet individual health needs. It promotes quality and cost-effective outcomes, acts as a liaison to physicians and members, and includes determining member assignment of treatment, pre-admission, and post-discharge counseling for acute conditions. The position also involves establishing relationships with members, handling immediate post-discharge follow-up, providing education and local resource information, and encouraging member self-education. Functioning in a clinical care advisory role, the nurse assesses members for wellness education and disease management, introduces members to website tools, educates on wellness and specific conditions, and facilitates care coordination. The role provides oversight to Holistic Health Coordinators.

The offered compensation varies based on skills, education, and experience. This role may be eligible for an annual incentive bonus, subject to plan terms.
