Job summary

Primary Health Care, Inc (PHC) in Des Moines, IA, seeks a Medical Assistant Care Navigator to support HIV care delivery under a Ryan White funded grant. You will identify and resolve barriers to care and provide administrative and clinical support as delegated within MA scope.

You will work with a multidisciplinary team on the PHC University Campus in Des Moines, typically 40 hours weekly, Monday–Friday 8 AM–5 PM. The role includes patient coordination and procedural assistance.

Qualifications

• High school diploma or equivalent required.
• One year of experience in a medical assistant, patient care technician, certified nurse assistant, phlebotomy, or related clinical position or completion of medical assisting program from an accredited institution.
• Effective clinical skills related to the position's essential functions.

Responsibilities

• Collaborates with the assigned patient care team to coordinate care for high-acuity patients living with HIV.
• Identifies and problem-solves barriers that prohibit or delay patients from receiving necessary care.
• Addresses patient inquiries and care needs in clinic and via EMR messaging.
• Coordinates referrals to outside service providers and completes prior authorizations as needed.
• Communicates with referral coordinators and HIV care teams to coordinate care.
• Facilitates patient rooming and intake, obtains vital signs, and prepares for exams/procedures.
• Performs point-of-care testing per CLIA guidelines and follows established workflows.
• Administers medications, immunizations, and treatments per orders.
• Documents service units and patient data in EMR per grant requirements and PHC policies.
• Performs quality controls and maintains infection control procedures; restocks supplies.

Job description

As a Medical Assistant Care Navigator, you will work collaboratively with a multidisciplinary team to support the delivery of quality healthcare through a Ryan White funded grant program for patients living with HIV. You will Identifiy and resolve barriers to care, provide administrative support, and clinical care as delegated by licensed clinical staff within the scope of a Medical Assistant.

This position typically works at the PHC University Campus in Des Moines andis typically scheduled to work 40 hours per week: Monday - Friday 8 AM - 5 PM.

What You Will Do

Collaborates with the assigned patient care team, Referral Coordinators, and external service providers to coordinate care for high-acuity patients (e.g. low health literacy, transportation barriers, language barriers, etc.) living with HIV. Identifies and problem-solves barriers that prohibit or delay patients from receiving necessary care.

Addresses patient inquiries and care needs both in clinic and through patient communication channels (phone, EMR messaging, etc.), collaborating with appropriate care team members as needed.

Coordinates assigned referrals to outside service providers and completes necessary prior authorizations as needed.

Communicates with referral coordinators, service providers, HIV care teams to coordinate care for high- acuity patients (e.g. low health literacy, transportation barriers, language barriers, etc.)

Facilitates the patient rooming and intake process and obtains accurate patient vital signs per established workflow. Prepares for exam/procedure and assists provider as requested.

Performs point-of-care testing per CLIA guidelines and other established workflows. Ensures service orders are completed timely. Administers medications, immunizations, and treatments per written or standing orders.

Documents service units and patient data in EMR based on grant requirements and PHC policies & Procedure.

Completes and accurately documents quality controls per established process. Performs pre-cleaning and transport of contaminated instruments per infection control policies and procedures. Routinely evaluates supply levels in exam rooms, removing expired supplies and restocking as appropriate.

Qualifications You Need

Required

• High school diploma or equivalent.
• One year of experience in a medical assistant, patient care technician, certified nurse assistant, phlebotomy, or related clinical position or completion of medical assisting program from an accredited institution.
• Effective clinical skills related to the position essential functions.
• Effective verbal and written communication skills.
• Excellent interpersonal, teamwork and collaboration skills with ability for a team approach to care plus develop and maintain effective working relationships with a diverse group of individuals.

• Basic computer skills with proficiency using Microsoft Office applications and electronic health record system.

Licenses & Certifications

• Current Basic Life Support (BLS) for Healthcare Providers
• Current Mandatory Reporter Training: Child Abuse and Dependent Adult Abuse recognized by the State of Iowa
• Must be obtained within the Introductory Period if not current.

Preferred

• Work experience in a community health center or medical clinic.
• Effective verbal and written communication skills in Spanish.

Your related experience and skills determine your base pay. The typical hiring range for this position is $18.65 - 23.32 per hour. Candidates with extensive related experience may be considered for a higher pay rate up to the pay range maximum.
