Job summary

Coastal Bend Wellness Foundation Inc. seeks a Client Services Specialist to support HIV services with direct client care and clinical quality management. The role focuses on tying Ryan White standards to real-time service delivery, improving retention in care, and achieving viral suppression.

The position requires chart reviews, care coordination, and data-driven quality initiatives, while maintaining a client caseload and ensuring compliant documentation in the EMR.

Qualifications

• Bachelor’s degree or equivalent experience required.
• Minimum 2 years of case management or healthcare-related experience.
• Preferred experience with Ryan White programs or HIV care.

Responsibilities

• Provide direct case management and linkage to HIV medical care.
• Review client charts and assist with service plans and reassessments.
• Coordinate care with medical and behavioral health providers and community partners.
• Assist with referrals for medications, housing, transportation, and support services.
• Document all client encounters accurately in the EMR and maintain caseload.
• Support quality management efforts and contribute to HAB performance measures.
