Zusammenfassung

Jordan CRC / Phoenix Counseling Solutions is seeking a full-time Project Coordinator to manage the HIV/AIDS Project, guiding participants from outreach to recovery services with trauma-informed care.

You will work closely with the Project Director to screen, enroll, assess, and plan services, then connect participants to HIV prevention and testing, SUD treatment, and recovery supports across partner agencies.

Qualifikationen

• Bachelor’s or Master’s degree in social work, counseling, addiction studies, psychology, behavioral health, human services, public health, or a related field.
• Ohio LSW and/or LCDC III strongly preferred; other behavioral health or SUD credentials considered.
• 3+ years in substance use treatment, behavioral health, co-occurring disorders, treatment coordination, case management, intake, or related human services.
• Experience with screening, assessments, treatment plans, ISPs, referrals, and trauma-informed care.
• Experience with outreach, referral partnerships, multidisciplinary teams, and diverse or underserved populations.

Aufgaben

• Coordinate the project day to day from outreach through discharge.
• Screen, enroll, and plan services within scope of practice (SUD, mental health, trauma, HIV-risk).
• Connect participants to SUD, behavioral health, HIV prevention and medical care.
• Build community partnerships with treatment providers, HIV/AIDS organizations, hospitals, housing, and justice-system partners.
• Collaborate, document, and measure through treatment-team meetings, data collection, and reports.
