Job summary

Jordan CRC / Phoenix Counseling Solutions in Ohio is hiring a full-time Project Coordinator to run the day-to-day work of our HIV/AIDS Project, ensuring no participant falls through the cracks from outreach to recovery support.

You will guide participants from first contact through screening, enrollment, and service planning, connecting them to substance use, behavioral health, HIV prevention and testing, and coordinated care with partners and evaluators.

Qualifications

• Bachelor’s or master’s degree in social work or related field.
• Ohio LSW and/or LCDC III preferred; other behavioral health credentials considered.
• 3+ years in substance use treatment or behavioral health coordination.
• Experience with screening, assessments, ISPs, referrals, and trauma-informed care.
• Experience with outreach, partnerships, and diverse populations.

Responsibilities

• Coordinate project day to day with goals, timelines and deliverables.
• Manage participant flow from outreach through intake, service planning, treatment, follow-up and discharge.
• Track engagement, referrals, missed appointments, barriers and service gaps.
• Screen, enroll, and plan services including SUD, mental health, trauma, and HIV-risk screening.
• Coordinate clinical assessments and Individualized Service Plans reflecting needs and goals.
• Coordinate access to SUD, behavioral health, and HIV prevention services.
