Job summary

Cook Children\'s Health Care System in Fort Worth, TX seeks an HIM Documentation Integrity Analyst I to evaluate discharge records for completeness and accuracy according to Joint Commission and state/federal rules.

You will identify deficiencies, communicate with HIM teams, and help ensure compliance and data quality. Qualifications include an associate degree in HIM or equivalent with hospital office experience; proficiency in MS Office; Epic EMR preferred.

Qualifications

• Proficiency in Microsoft Office required. Epic EMR experience preferred.

Responsibilities

• Evaluates discharge records for inclusion, accuracy, and completeness per Joint Commission/state/federal requirements.
• Assigns chart deficiencies per CCMC Medical Staff Rules and Regulations.
• Communicates with HIM Records Management, Transcription and Quality staff to report missing/incorrect documentation.

Job description

The HIM Documentation Integrity Analyst I evaluates patient records upon discharge for inclusion, accuracy, and completeness of required documentation as defined by The Joint Commission, state, and federal regulations. Assigns chart deficiencies based on CCMC Medical Staff Rules and Regulations. Communicates with HIM Records Management, HIM Transcription and HIM Quality Compliance staff to report missing and incorrect document issues.

Qualifications:

• Must have an Associates degree in Health Information Management OR must have a High School Diploma or equivalent with two (2) years of hospital-centered office experience.
• Requires a broad knowledge of medical terminology, record management concepts, data integrity of patient related information, and HIM office applications.
• Must be proficient in Microsoft Office software.
• Must be detail- oriented, analytical, have strong organizational, writing/interpersonal communication skills, and the ability to maintain confidentiality.
• Must work well independently with minimal supervision and have the ability to solve problems appropriately using job knowledge and current policies and procedures.
• Epic EMR experience preferred.
• Experience with analysis preferred.
