General Purpose of Job:

Direct patient care to infants and children between the ages of 0 to 3 years in outpatient setting of former NICU graduates and other high-risk patients.Promotes medical nutrition therapy using evidence-based practice to optimize patient care. Provides consultation to the health care team to ensure efficient and effective nutrition care of patients. Educates patients, families, health care team, and members of the community.

  • Maintains utmost level of confidentiality at all times.
  • Adheres to hospital policies and procedures.
  • Demonstrates business practices and personal actions that are ethical and adhere to corporate compliance and integrity guidelines.
  • Works with medical staff and nursing staff to promote optimal patient care;
  • Attends patient care meetings when applicable.
  • Able to accurately obtain anthropometric measurements
  • Conducts a thorough nutritional assessment on high risk patients
  • Interviews patients/families and documents any pertinent information in the EMR
  • Formulates and determines a plan that is focused on the etiology of the problem and known to be effective based on the current knowledge of evidence
  • Develops nutrition care plans for patients at nutritional risk and assures implementation and evaluation of care plans, including recommendations for nutrition intervention and education.
  • Reviews nutrition care plan with the medical provider
  • Counsels patients and families on the plan of care.
  • Utilizes medical history, lab results, anthropometric measurements and diet history to evaluate nutrition related consequences
  • Monitor patients tolerance to treatment modalities and progress through checking patient understanding and compliance with plan; determines if the plan is being implemented as prescribed; Provides and gathers information indicating reasons for lack of progress and supports conclusion with evidence.
  • Evaluates outcomes by comparing current finding with previous status, intervention goals, and or reference standards.
  • Documents relevant, accurate, and timely information in the medical chart in accordance with department standards.
  • Coordinates with appropriate hospital staff the need for continuum of care
  • Interprets and presents current clinical knowledge, standard of care, and research to improve patient care and nutrition service line
  • Serves as an educator and resource to the medical staff and allied health personnel on nutrition issues, including nutrition support therapy principles, content, and issues as needed for specific patient populations.
  • Participates in the training and orientation of new staff
  • Develops current education materials.
  • Ensures formula mixing directions are up to date
  • Maintains and accountable for Formula room as per compliance standards
  • Inputs charges as appropriate in hospital computer system
  • Coordinates with leadership team on all aspects of patient care.
  • Monitors the departments Quality Improvement Programs and assists the department in initiating corrective action.
  • Completes all applicable forms associated with patient nutrition
  • Must be proficient in reading, writing and speaking Spanish.
  • Weekly travel to satellite clinic locations
  • Telemedicine services to all locations
  • Performs other duties as required.

Education and/or Experience:

  • Bachelor's degree (B. S.) or Master (M.S.) and successful completion of a Coordinated Undergraduate Program (CUP) or Internship Program
  • Registered (RDN) by the American Dietetic Association required
  • Licensure (LD) by the State of Texas required.
  • New graduates to obtain Registration within 6 months of hire; Licensure by the State of Texas within 3 months of hire date.
  • Zero-3 year’s dietitian experience.

Certificates, Licenses, Registrations:

  • Registered by the American Dietetic Association, Licensed by the State of Texas, (If RD Eligible, employee must obtain registration within 6 months of date of hire.)
