Job summary

Personal Healthcare Management LLC is seeking a licensed Physical Therapist in Massachusetts for a SNF setting. You will evaluate residents, develop and modify PT plans of care, supervise PTAs, and collaborate with the interdisciplinary team to meet rehab goals.

Position requires MA PT license, Bachelor's degree in Physical Therapy, and preferred geriatric experience. Valid MA driver's license and ability to use clinical software are expected.

Qualifications

• Must possess a current unencumbered MA license as a Registered Professional Physical Therapist.
• Bachelor’s degree or higher in Physical Therapy.
• Geriatric experience preferred.
• A current unrestricted driver’s license and dependable transportation.
• Able to use necessary computer applications.

Responsibilities

• Screens residents for rehabilitation therapy needs.
• Completes Physical Therapy evaluations upon physician order.
• Establishes PT plan of care for treatments per physician order.
• Provides PT plan of treatments and modalities.
• Modifies treatment program level, intensity and techniques based on resident response.
• Documents resident progress toward PT goals.
• Discharges residents from PT services when appropriate.
• Makes recommendations for nursing regarding mobility, positioning and ROM needs.
• Completes daily activity schedule, progress notes and re-certifications per facility policy.
• Supervises PT assistants and unlicensed assistive personnel.
• Provides education to residents, staff and families.
• Demonstrates knowledge of Medicare Part A PPS case management.
• Maintains compliance with regulations and facility policies.
• Uses clinical software per policies and procedures.
• Completes care plans and MDS as directed.
• Participates in discharge planning with interdisciplinary team.
• Participates in facility meetings and committees as assigned.
• Submits reports in established time frames.
• Participates in internal audits as directed.
• Understands Medicare/Medicaid funding sources in SNF setting.
• Maintains professional knowledge and confidentiality.
