The role includes evaluating residents, developing treatment plans, supervising staff, documenting progress, and coordinating with the interdisciplinary team to optimize mobility and quality of life. Proficiency with Medicare rules and strong communication are essential for success.

Qualifications

• Must have a current unencumbered MA license as a Registered Professional Physical Therapist.
• Graduate of an accredited school of physical therapy or international equivalent.
• Geriatric experience preferred.
• A current unrestricted driver’s license and dependable transportation.
• Able to use necessary computer applications.

Responsibilities

• Screens residents for rehabilitation therapy needs.
• Completes Physical Therapy evaluations upon physician order.
• Provides Physical Therapy plan of treatments and modalities.
• Modifies treatment program level, intensity and techniques based on the resident’s response to specific treatment plan.
• Documents resident progress toward Physical Therapy goals.
• Discharges residents from Physical Therapy services when appropriate.
• Makes recommendations for nursing regarding mobility, positioning and ROM resident needs.
• Completes Daily Activity Schedule, progress notes and re-certifications per facility policy.
• Supervises Physical Therapy Assistants and unlicensed assistive personnel.
• Provides resident, staff and family education and in-servicing.
• Demonstrates knowledge of Medicare Part A PPS case management.
• Complies with Federal and State regulations related to rehabilitation therapy.
• Completes care plans and MDS as directed, per facility policy.
• Participates in discharge planning with the interdisciplinary team.
