Clarvida in Santa Ana, CA is seeking a High Facility Wrap Facilitator 1 | 2 to coordinate services for children, youth, and families in the High Fidelity Wraparound program.

You will develop care plans, connect clients to resources, and support long-term success within homes, schools, and communities, while serving as the primary contact for clients and families.

• Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Behavioral Sciences, or a related field.
• Minimum of four years of experience in behavioral health, social services, child welfare, case management, or a related field.
• Up to two years of graduate education may be substituted on a year-for-year basis for experience requirements.
• Preferred 5150 Certified

About the Role
High Facility Wrap Facilitator 1 | 2 is responsible for coordinating services and supports for children, youth, young adults, and families enrolled in the High Fidelity Wraparound (HFW) Program. HFW is an intensive, team-based care coordination model designed to support individuals with complex behavioral health needs who may be involved with multiple systems, including mental health, education, child welfare, juvenile justice, and housing services. The PSC serves as a primary point of contact for clients and families, helping to develop care plans, coordinate services, connect individuals to resources, and support long-term success within their homes, schools, and communities.

Productivity Expectation: This position has a baseline productivity expectation of 50%, generally equivalent to an average of approximately 3.5 hours of direct client service per workday. The meeting of such requirements will be addressed in accordance with the Company policies and applicable law.

Responsibilities

Personal Service Coordinator

• Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Behavioral Sciences, or a related field
• Minimum of four years of experience in behavioral health, social services, child welfare, case management, or a related field
• Up to two years of graduate education may be substituted on a year-for-year basis for experience requirements
• Experience coordinating care for individuals with complex behavioral health needs preferred
• Intermediate computer proficiency
• Valid California driver’s license, reliable transportation, and active automobile insurance
• Preferred 5150 Certified

Preferred Qualifications

• Experience working with children, youth, and families
• Experience in Wraparound, Full Service Partnership (FSP), case management, or care coordination programs
• Knowledge of behavioral health systems, rehabilitation models, and community resources
• Bilingual skills preferred
• Experience working in community-based behavioral health settings
