Job summary

A List Therapy Group in California is seeking a Home Health Occupational Therapist (OT) to provide high-quality in-home occupational therapy services under a referral network. You will evaluate patients, develop and update care plans, and coordinate with physicians and families to support safety and independence.

This contractor position offers flexible scheduling and autonomy over caseload, with earnings potential of $150,000–$180,000 annually and $85–$110 per completed visit, plus a $250

Responsibilities

• Provide occupational therapy services in home health settings in accordance with law, physician orders, payer requirements, and plan of care.
• Evaluate patients in their homes and provide skilled OT interventions to support safety, independence, ADLs, and quality of life.
• Develop, update, and implement care plans aligned with clinical judgment and payer requirements.
• Assess home environments and recommend equipment, modifications, safety strategies, or caregiver education as appropriate.
• Communicate with referring agency, physician, patient, family, and other care participants as permitted.
• Document patient progress accurately and efficiently per payer, agency, and regulatory requirements.
