Qualifications

• Active, unencumbered LPN/LVN license in the practicing state
• CPR certification (we can assist after hire if needed)
• TB skin test within last 12 months
• Six months of prior hands-on nursing experience preferred
• Reliable transportation

Position Details

Location: Telford, PA - Home setting

Schedule: Day and overnight shifts available

Patient Age Range: <12 months

Acuity: GT, Trach, Vent

Responsibilities

• Deliver and document skilled nursing care according to care plans and physician orders
• Assess patient condition continuously and report changes
• Develop, implement, and update nursing care plans
• Recognize and respond to changes in patient needs
• Maintain compliance with policies, regulations, and standards
• Educate patients and families on disease management and self-care

Requirements

• Graduate from an accredited nursing program
• Current, unrestricted RN license
• Current CPR certification
• Proficiency in assessments, documentation, and compliance
