Job description

Responsibilities

• Performs professional nursing services requiring the application of the biological, physical and social sciences and nursing skills in the care of the sick, the prevention of disease, and the conservation of health.
• Works in an interdisciplinary relationship with other members of the corporation's staff to provide coordinated Home Health services to individuals and their families in accordance with a Plan of Care established with the medical provider, patient and family.
• Assumes primary case management functions for assigned patients.

Qualifications

• Completes and maintains all competencies and trainings as required for the role.
• Graduate of an accredited school of Nursing.

Experience

• Relevant experience in home health or related nursing field.

Certification and Licensure

• Current valid driver’s license.
• PA Registered Nurse license.
• BLS certification obtained within 60 days of hire date.
