Personal Touch Home Aides of New York is seeking an HHA Coordinator- CHHA in Brooklyn, NY. This full-time, in-person role requires prior HHAeXchange experience and knowledge of EVV, Medicaid compliance, and scheduling within a home care setting.

The HHA Exchange Coordinator is responsible for managing caregiver records and schedules within the HHAeXchange platform, ensuring compliance with regulatory requirements, supporting EVV (Electronic Visit Verification) processes, and serving as a liaison between caregivers, coordinators, payers, and internal departments. This position plays a key role in maintaining accurate data, resolving system issues, and supporting efficient service delivery.
Maintain accurate caregiver, patient, and authorization records in HHAeXchange.
Monitor and resolve scheduling, visit verification, and billing exceptions.
Process caregiver onboarding and profile updates within the system.
Ensure authorizations, service codes, and payer information are entered correctly.
Monitor EVV compliance and address missing or incomplete visit records.
Investigate and resolve visit exceptions, missed punches, and documentation discrepancies.
Work with caregivers and coordinators to correct attendance and visit verification issues.
Generate and review EVV compliance reports.
Assist with caregiver scheduling and shift coverage as needed.
Verify service hours against authorizations and approved care plans.
Coordinate schedule adjustments and communicate changes to stakeholders.
Ensure documentation meets payer, Medicaid, and agency requirements.
Conduct routine audits of caregiver and patient records.
Track expiring credentials, licenses, and mandatory training requirements.
Support internal and external audits.
Prepare operational, compliance, and performance reports.
Serve as a point of contact for caregivers regarding HHAeXchange-related questions.
Collaborate with Intake, Billing, Payroll, Clinical, and Operations teams to resolve issues.
Communicate effectively with managed care organizations (MCOs) and payer representatives.
High school diploma required; Associate's or Bachelor's degree preferred.
1-3 years of home care, healthcare operations, scheduling, or EVV experience.
Experience working with HHAeXchange preferred.
Strong knowledge of Medicaid and home care compliance requirements.
Proficiency in Microsoft Office Suite, particularly Excel.
Excellent organizational, communication, and problem-solving skills.
Ability to manage multiple priorities in a fast-paced environment.
This position typically operates in an office environment.
Prolonged periods of sitting and computer work.
Ability to communicate effectively by phone, email, and virtual platforms.
