Job description

AscendCare Home Health Solutions LLC is seeking compassionate and dependable Caregivers/Home Health Aides (HHAs) to join our team and provide high-quality care to clients in their homes. Our caregivers play a vital role in helping individuals maintain dignity and independence while receiving supportive services in the comfort of their familiar surroundings.

As a Home Health Aide, you will assist with personal care, light housekeeping, and companionship to clients in need, under the supervision of a Registered Nurse.

Key Responsibilities:
  • Provide personal care assistance including bathing, grooming, dressing, and toileting
  • Assist with ambulation, transfers, and mobility support
  • Medication administration, reminder, assistance
  • Prepare and serve meals; assist with feeding as needed
  • Perform light housekeeping tasks such as laundry, dishes, and tidying
  • Offer emotional support and companionship
  • Monitor and report changes in client condition to the RN supervisor
  • Follow individualized care plans and document services provided
  • Transportation, errands, shopping
Qualifications:
  • High school diploma or GED Completion of a Home Health Aide, CNA, or PCA training program
  • Must be at least 18 years of age
  • Ability to pass a criminal background check and TB screening
  • Reliable transportation
  • Strong communication and interpersonal skills
  • Compassionate, patient, and dependable
  • Willingness to learn
  • Respectful of clients, client's homes, and their wishes/rights
Preferred Qualifications:
  • Previous home care experience
  • Experience working with seniors or individuals with disabilities
  • Culturally sensitive and respectful of client preferences
  • Familiarity with dementia care
  • CPR and First Aid Certified (will train)
  • Experience with medication: administration, reminders, and assistance
  • Experience documenting in Electronic Health Record System (Ex: Point Click Care, Well Sky, Eldermark)
Schedule & Work Type:
  • Part-time and PRN (as needed) shifts
  • Weekday and weekend availability
