DUTIES
To provide nursing care, in accordance with the patient's plan of care, including comprehensive health and psychosocial evaluation, monitoring the patient’s condition, health promotion and prevention, coordination of services, teaching and training activities, and direct nursing care.
Responsibilities
- Coordinate total patient care by conducting comprehensive health and psychosocial evaluations, monitoring the patient's condition, promoting preventive practices, coordinating services, and teaching/training activities.
- Evaluate the effectiveness of nursing services to the patient and family on an ongoing basis.
- Perform admission, transfer, re-certification, resumption of care, and discharge OASIS assessments for home care patients.
- Prepare and present patient records to the Clinical Record Review Committee as indicated.
- Consult with the attending physician regarding alterations of Patient Care Plans and make changes as appropriate.
- Coordinate patient services and submit daily tallies of patient care visits.
- Participate in case conferences and discuss patient issues with supervisors.
- Discuss the need for involvement of other health team members and coordinate their services.
- Cooperate with other agencies to ensure continuity of care.
- Participate in staff development, in-service education, workshops, and professional organizations.
- Develop and revise the physician's Plan of Treatment and process change orders as needed.
- Submit clinical notes within 48 hours and maintain comprehensive documentation.
- Participate in discharge planning and stay updated on current drug therapies.
- Adhere to federal, state, and accreditation requirements, including Medicare and Medicaid regulations.
- May be requested to fill in for other nurses as needed.
- Coordinate admission of a patient, including initial assessment and ongoing evaluations.
- Develop and implement nursing care plans and evaluate patient needs.
- Provide skilled nursing care as outlined in the care plan, including treatments and preventative procedures.
- Supervise and evaluate care provided by the Home Health Aide, including supervisory visits and evaluations.
- Discuss patient care with the supervisor and involve other health team members as needed.
- Obtain orders for home health aide services and provide guidance to staff.
- Participate in educational and planning activities for nursing services.
- Prepare care plans for Home Health Aides.
- Job conditions include having a driver’s license, the ability to access patients' homes, and physical capability to perform assessments and care activities.
- Operate equipment such as thermometers, B/P cuffs, glucometers, and others.
Qualifications
- Graduate from an accredited School of Nursing.
- Licensed in Florida as a Registered Nurse.
- One or more years of experience preferred, in community/home health or hospital settings.
- Knowledge of Medicare and Medicaid guidelines.
- Understanding of home healthcare principles and documentation.
- Organizational and time management skills.
- Ability to contribute to quality care through communication with management and staff.
- Criminal background check required.
- Current CPR certification (online not accepted).
