Hera empowers clinicians to deliver dedicated care management for older adults in the community, fully remote and flexible. You will set your own schedule as a 1099 provider, working 30–40 hours per week with compensation in the $30–$60 per hour range.

Ideal candidates hold LMSW, LCSW, or RN credentials and have geriatric care management experience. You should be tech‑savvy, adaptable to evolving tools, and passionate about building connections with families.

Qualifications

• Experience in geriatric care management, hospice, palliative care, discharge planning, rehab, or hospital social work.
• Ability to manage a remote client load with autonomy.
• Comfort with AI tools to enhance care coordination.

This is not a typical social work or nursing role. If you've felt that traditional healthcare roles have a ceiling on you, and you want to actually bridge the gaps for older adults after they leave the office or facility, Hera is for you.

Hera is dedicated care management in the community for older adults, covered by Medicare. Every family gets matched with a dedicated Hero, you, who actually has the tools, time, and autonomy to help. It's not a 15 min monthly check-in call. It's real, ongoing support where you see things through and families call you their Hero.

The Role

• 1099, 30-40 hrs/week in business hours (you set your schedule)

• $30-60 / hour: factors include experience, geography, and client load

• Serve older adults and their families in aging at home safely

You'd be a great fit if you are

• Hold an LMSW, LCSW, or RN (not required but a plus)

• Have experience in geriatric care management, hospice, palliative care, discharge planning, rehab, or hospital social work
