Job summary

Cleveland Clinic West Palm Beach is seeking a Care Coordinator in Hepatology to partner with a multidisciplinary team to coordinate comprehensive care for patients with chronic liver disease. The role emphasizes patient education, disease management, and transitions of care to improve outcomes.

The RN will work Monday through Friday, 8:00 a.m. – 5:00 p.m., collaborating with hepatology specialists and inpatient teams to reduce readmissions and optimize care across settings.

Qualifications

• Graduate from an accredited school of professional nursing.
• Current state RN licensure.
• BLS certification (AHA).
• Three to five years of nursing experience.
• Preferred BSN.
• Specialty certification or care coordination experience encouraged.

Responsibilities

• Work with a multidisciplinary care team across the continuum to coordinate care.
• Assess and manage transitions of care to optimize outcomes and reduce readmissions.
• Educate patients and families on disease management and self-care.
• Identify high-risk patients and coordinate plan with primary/specialty teams, social work, pharmacy and others.
• Utilize care registries and tools to manage patient populations and track progress.
