Job description

Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

1. Assess patient status and implement care plans with the interdisciplinary team
2. Coordinate care and educate patients
3. Administer medications and treatments as directed
4. Monitor and document patient responses to treatment
5. Mobilize resources in complex cases to enhance patient recovery
6. Integrate assessment data to anticipate healthcare needs
7. Ensure effective handoff communication
8. Manage staffing and develop individualized comfort measures
9. Participate in performance improvement activities
10. Perform other duties as assigned

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification
• Clinical nursing experience in dialysis preferred

Rate: $45.00 - $55.00
