Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

1. Assess patient status and monitor care in collaboration with the interdisciplinary team.
2. Coordinate care and educate patients.
3. Administer medications and treatments as directed.
4. Monitor and document patient responses during treatment.
5. Mobilize resources in complex cases to maximize patient participation in recovery.
6. Use assessment and diagnostic information to anticipate healthcare needs.
7. Ensure effective handoff communication and reporting.
8. Manage staffing patterns proactively.
9. Provide individualized comfort measures based on evidence-based practices.
10. Participate in performance improvement activities.
11. Perform other related duties as assigned.

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification
• Clinical nursing experience in dialysis preferred

Rate: $45.00 - $55.00
