Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

1. Collaborate with the interdisciplinary team to assess patient status and implement and monitor care plans.
2. Coordinate care and educate patients.
3. Administer medications and treatments as directed by physicians.
4. Monitor patients during treatment and document responses.
5. Mobilize resources in complex cases to maximize patient participation in recovery.
6. Integrate assessment data with intuition to anticipate healthcare needs.
7. Ensure effective handoff communication and report on potential issues.
8. Manage staffing patterns proactively.
9. Provide individualized comfort measures using evidence-based practices.
10. Participate in performance improvement activities.
11. Perform other related duties as assigned.

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification
• Clinical nursing experience in dialysis preferred

Rate: $45.00 - $55.00
