Job summary

Cleveland Clinic Health System in Port Saint Lucie, FL is seeking a Pharmacy Clinical Specialist in Hematology to coordinate pharmaceutical care for an assigned clinical unit and participate in patient care rounds with medical staff. The role requires a PharmD or BS in Pharmacy with completion of a post-graduate residency or fellowship; Florida licensure; and ongoing education.

Preferred PGY2/fellowship; board certification is encouraged.

Qualifications

• PharmD or Bachelors in Pharmacy with post-graduate residency or fellowship training required.
• If hired for a specialty area where specialty residency training is available, then specialty residency is required.

Responsibilities

• Coordinates and provides pharmaceutical care for patients on an assigned clinical unit and participates in patient care rounds with medical staff.
• Performs pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in the service area, considering age and clinical condition.
• Provides information to patients, physicians, nurses and other health care professionals.
• Supports education of staff involved in providing pharmaceutical care to patients.
• Supports research in the area of specialty.
