Job summary

Duke University Health System is seeking an RN Care Coordinator for the Pediatric Hematology/Oncology department at Duke Children's Health Center in Durham, NC. You will collaborate with providers, nurses, and care teams to coordinate complex visits, pre-visit assessments, and clinic flow, while delivering telephone triage and clinical guidance to families.

Requires a BSN, NC RN licensure, and 3 years of pediatric nursing experience; hematology/oncology experience is preferred.

Qualifications

The Children's Health Center at Duke University Hospital is comprised of pediatric outpatient clinics. Our team is active in delivering exemplary care for each patient to ensure an excellent patient experience, every time. The Pediatric Hematology / Oncology Department at the Duke Children's Health Center is active in delivering exemplary care for each patient to ensure an excellent patient experience. The Pediatric Hematology / Oncology RN Care Coordinator will provide triage support and care coordination for this high-acuity patient population in collaboration with dynamic multidisciplinary teams. The role establishes rapport with patients and families by phone, coordinates services, and provides education and nursing support to patients and families in and out of the clinic setting.

Schedule

• Full Time: Hours may vary, but will typically fall within a standard Monday-Friday, 8 a.m.-5 p.m. schedule.

How you will make an impact

• Collaborate with providers, case managers, registered nurses, and other members of the health care team to assess patient needs, develop plans of care, and optimize outcomes for complex patient populations.
• Provide telephone triage and clinical consultation across multiple clinical areas; monitor outcomes, document patient responses to care, and keep providers informed.
• Serve as a point of contact for patients, families, and care teams; provide information on recommended treatments and coordinate complex visits, pre-visit assessments, and clinic flow.
• Coordinate prescription refills, prior authorizations, medications, coverage options, treatment equipment, and related clinical paperwork, including lab, school, leave, medical release, emergency, travel, disability, and discharge forms.
• Educate patients and caregivers about access to care, disease states, treatments, procedures, medications, home care, and continuation of care, and connect them with internal, external, and community resources.
• Coordinate internal and external referrals, future appointments, post-hospitalization care, home discharge planning, and transitions with inpatient and hospital care coordination teams.
• Support patients and caregivers with information about service costs, insurance coverage, payment options, and access to continuing care resources.
• Participate in quality assurance and performance improvement activities; support best practices, regulatory compliance, timely documentation, reporting, and record maintenance.

What you will need

• BSN required.
• 3 years of nursing experience required.
• Current or compact RN licensure in the state of North Carolina required.
• Solid background in pediatric nursing; hematology/oncology experience preferred.
• Experience with telephone triage or managed care coordination.
• Strong oral and written communication skills and software navigation abilities; EPIC experience preferred.
• Good organizational skills.
• Flexible attitude with the ability to function independently and as a team member while assisting patients and families through home care coordination and in the clinic setting.
• Knowledge of the registered nurse scope of practice and appropriate application of the nursing process.
• Ability to independently seek resources, work collaboratively, and establish and maintain effective working relationships.
• Ability to communicate clearly with patients, families, visitors, the health care team, physicians, administrators, leadership, and others.
• Ability to teach patients and families in accordance with the nursing plan of care.
