Zusammenfassung

The Christ Hospital Health Network is seeking a Quality & Performance Coordinator to drive improvements in HEDIS measures and value-based care. You will conduct outbound member calls, coordinate with medical offices, and support NCQA and CMS compliance to sustain a 5-star performance level.

Responsibilities include data analysis in Epic dashboards, care-gap closure, and collaboration with internal teams and payer partners to optimize care delivery and preventive services for patients.

Qualifikationen

• Experience with HEDIS measures and quality programs.
• Strong phone communication and problem-solving ability.
• Familiarity with payer programs and data concepts.
• Ability to work independently and with cross-functional teams.

• Analyze HEDIS performance and track progress against targets.
• Coordinate with providers to close care gaps and improve adherence.
• Review medical records and abstract data for NCQA measures.
• Engage members and providers to promote preventive services.
• Ensure compliance with HIPAA, NCQA, and CMS requirements.
