Responsibilities

• Extract data from medical record sources to gather information and perform outreach.
• Communicates details to the appropriate team, providers, and/or supervisors.
• Coordinates a wide range of activities and education to support the patient population using telehealth, face-to-face, text, telephone, or e-mail.
• Utilizes reports to prioritize outreach to ensure care coordination.
• Works with physicians and multidisciplinary care management teams; facilitates care coordination and documents efforts.
• May assist with obtaining authorizations and scheduling where appropriate.
