Qualifications

  • ACLS within 1 year as required by AHA.
  • Licensed Registered Nurse (RN) in Illinois.
  • BLS within 30 days as required by AHA.
  • Bachelor’s Degree in Nursing and College Diploma in Nursing.

Responsibilities

  • Assists patients in understanding diagnosis, treatment options and resources.
  • Serves as liaison in planning care between patients, caregivers and multi-disciplinary health care providers.
  • Provide education and informational resources to patient to optimize continuous and comprehensive care.
  • Provides health and wellness education to individuals and groups.
  • Acts as preceptor and supports the development of other staff and healthcare students.
  • Provides leadership for initiatives designed to improve the quality of care or clinical outcomes.
  • Attends daily inpatient huddles to provide input and address care and educational needs.
