Job summary

CentraCare Health's St. Cloud Hospital is seeking a Heart Failure Registered Nurse to join a collaborative care team. The role focuses on education, support, and ongoing disease management for patients with heart failure across the continuum of care.

The position involves bedside hospital care, management of new and follow-up visits in the outpatient clinic, and occasional remote work from home. A BSN or MSN is preferred and Minnesota RN licensure is required.

Qualifications

• Graduate from an accredited nursing program.
• Current Minnesota RN license required.
• BSN or MSN preferred.
• BSN or higher required within 36 months of hire.
• BLS Healthcare Provider certification required.
• National Certification in Advanced Heart Failure preferred (within 24 months).
• 1 year of recent cardiology work preferred.
• 2 years in a cardiovascular disease program preferred.
• Good written and verbal communication skills.

Responsibilities

• Provide bedside hospital care.
