Qualifications

• Participate in patient safety initiatives.

Responsibilities

• Coordinate education for staff and provide community education lectures.
• Communicate effectively with physicians and chair the Heart Failure multidisciplinary team.
• Plan and provide cardiac and pulmonary rehabilitative care, ensuring patient safety.
• Conduct individual needs assessments and establish exercise prescriptions and risk-factor reduction plans.
• Support Anaheim Regional Medical Center’s strategic plan, goals, and performance improvement plan.
• Support organizational expectations including Customer Service and Patient's Rights.

Skills

Effective oral and written communication skills

Ability to provide individual and large group counseling sessions

Experience in cardiology nursing

Experience with data collection

Education

RN from an accredited nursing program

Three years of clinical health care or health education experience

Tools

BLS certification
