Job summary

US Heart and Vascular is seeking a Cardiac Rehabilitation Nurse Coordinator to manage the Cardiac Rehabilitation and Intensive Cardiac Rehabilitation programs in Houston, TX. You will oversee patient enrollment, assessments, education, and coordination with physicians and the rehabilitation team.

The role focuses on monitoring patients during exercise, conducting telemetric assessments, and promoting evidence-based risk reduction to improve functional capacity and reduce readmissions.

Qualifications

• BSN from an accredited school of nursing.
• Current RN license in state of practice.
• Current BLS and ACLS certification.
• CCRP is preferred but not required.

Responsibilities

• Coordinate daily operations of the Cardiac Rehabilitation and Intensive Cardiac Rehabilitation programs.
• Enrol patients, schedule, orient, and progress through the program.
• Perform comprehensive nursing assessments and document medical history & risk factors.
• Monitor telemetry, respond to emergencies, and maintain ACLS readiness.
• Educate patients on risk reduction, lifestyle changes, and adherence to therapy.
