Responsibilities

• Lead Heart Failure program quality improvement and readmission reduction initiatives in partnership with physician and operational leadership.
• Supervise and mentor a team of 9 Advanced Practice Providers (APPs), including performance management, professional development, and annual evaluations.
• Oversee APP scheduling, PTO management, staffing coverage, and workflow optimization across ambulatory and virtual heart failure services.
• Collaborate with physicians, nursing leadership, care coordination teams, and specialty programs to enhance patient access, care transitions, and operational efficiency.
• Review routine and urgent referrals, support complex patient care needs, and coordinate multidisciplinary care for advanced heart failure and LVAD patients.
