MultiCare Pulse Heart Institute is seeking a Heart Failure Nurse Practitioner to join our busy cardiovascular practice in Washington. The position requires coordinating care for decompensated heart failure in an outpatient setting and collaborating with social work, cardiac rehab, and palliative care teams, with ED referrals as needed.

Qualifikationen

Masters of Science in Nursing.
Post Bachelor's Nurse Practitioner program.
Certified Nurse Practitioner exam completion.
WA ARNP licensure by start of employment.
DEA, NPI and prescriptive authority.
Current BLS and ACLS per American Heart Association.
ARNP experience in Heart Failure or 2+ yrs in cardio setting.

Aufgaben

Manage decompensated heart failure in outpatient clinic.
Collaborate with social work, cardiac rehab, and palliative care teams.
Refer to the ED when clinically indicated.
