Job summary

Intermountain Health is seeking a Registered Nurse for specialty care management at the Billings Downtown Clinic Cardiology. You will assess, plan, facilitate, and advocate for patients, collaborating with families, physicians, and the care team to coordinate services across the continuum.

Preferred candidates have Cardiology experience, Heart Failure expertise, and a BSN within three years. This full-time role offers a generous benefits package and a 40-hour workweek, with flexible daytime

Qualifications

• Licensed RN in the state of practice (Montana)
• Cardiology experience preferred
• Heart Failure experience strongly preferred
• BSN required within 3 years of hire/promotion (Nevada exception)

Responsibilities

• Apply Case Management Standards of Practice (Assess, Plan, Facilitate, Advocate) to assigned patient population
• Assess patient/family needs and barriers to care and collaborate with physicians and team
• Develop and implement a treatment plan to improve outcomes
• Facilitate communication among family and care team and document interventions
• Coordinate with physicians, patients, and staff to ensure seamless care
• Lead regional/system-wide forums to improve processes and materials
• Track activities using standardized databases and tools
