Job summary

Qualifications

• Three years general nursing experience.

Responsibilities

• Conducts new patient interviews and assesses patients' functional health and educational needs.
• Monitors patients' labs, test results and progress.
• Provides patient/family education.
• Coordinates discharge needs for smooth transition from hospital to outside agencies.
• Acts as a liaison between patient and other medical providers regarding progress and complications.
• Collaborates with multi-disciplinary teams for assessing and evaluating patient care.
• Participates in continuing education and in-service programs.
