Job summary

Endeavor Health’s Center for Cardiac Health–Heart Failure team seeks an Advanced Practice Provider (APN/PA) to deliver autonomous, patient-centered care for high-acuity cardiac patients in Elmhurst, IL. The role combines clinical leadership with collaboration under a supervising physician.

Full-time option with 40 hours/week, located at Elmhurst Hospital. Requires IL APN/PA licensure, DEA, and master’s-level training in a cardiology specialty; BLS/CPR is mandatory and prior cardiology experience

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in IL required.
• Federal DEA and Illinois Controlled Substance License required.
• Education: Completion of master’s level program in specialty area.
• Certification: BLS/CPR certification required.
• Experience: Prior APN/PA Cardiology experience required.

Responsibilities

• Obtaining comprehensive patient histories and performing physical examinations.
• Ordering and interpreting diagnostic tests.
• Developing differential diagnoses and prescribing evidence-based treatments.
• Managing acute and chronic conditions.
• Providing ongoing education to patients and families.
• Coordinating care with physicians and interdisciplinary teams to optimize outcomes.
• Supporting quality-improvement and patient-safety initiatives.
• Contributing to organizational goals and service-line strategies.
