Endeavor Health is seeking an Advanced Practice Provider (APN/PA) for the Center for Cardiac Health - Heart Failure in Elmhurst, IL. The role is full-time with an outpatient focus, Monday–Friday hours, and a 40-hour week.

You will obtain patient histories, perform exams, order and interpret tests, manage acute and chronic conditions, educate families, and coordinate care with physicians and interdisciplinary teams to optimize outcomes and post-discharge follow-up.

Qualifications

• APN or PA licensure in IL required
• DEA license required
• BLS/CPR certification required
• Cardiology/APN or PA experience required

Hourly Pay Range:

$56.25 - $72.12 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

• Position: Advanced Practice Provider (APN/PA) – Center for Cardiac Health - Heart Failure
• Location: Elmhurst Hospital, Elmhurst, IL
• Full Time/Part Time: Full-Time (average of 40 hours/week)
• Hours: Monday – Friday between 7:00AM – 5:30PM.
• Required Travel: n/a
• Outpatient role

Job Description:

• Obtaining comprehensive patient histories and performing physical examinations
• Ordering and interpreting diagnostic tests
• Developing differential diagnoses and prescribing evidence-based pharmacological and nonpharmacological treatments
• Managing acute and chronic conditions
• Providing ongoing education to patients and their families
• Coordinating care with physicians and interdisciplinary teams to optimize outcomes and support post-discharge follow-up and transitional care
• Supporting quality-improvement and patient-safety initiatives
• Contributing to the development and implementation of organizational and service-line goals
• Adhering to all Endeavor Health policies, procedures, and standards.

Qualifications:

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required

• Education: Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Prior APN/PA Cardiology experience required

Special Knowledge and Skills Required:

• APP experience in cardiology required

Preferred Qualifications:

• Experience working with Heart Failure patients preferred
