Responsibilities

• Coordinate the Healthy Families America program and support fidelity to Best Practice Standards.
• Develop and monitor program goals, objectives, and policies.
• Support accreditation readiness, quality assurance, and continuous quality improvement.
• Recruit, hire, train, supervise, coach, and develop program staff.
• Promote reflective supervision and manage caseload assignments and workload distributions.
• Maintain an assigned caseload and conduct home visits per HFA requirements.
• Partner with families to develop Family Goal Plans and track progress toward goals.
• Provide education on parenting, child development, prenatal and maternal health, and family wellness.
• Complete screenings and connect families with healthcare, housing, education, and social services.
• Develop Family Resource Center activities aligned with program goals.
• Oversee data collection, documentation, reporting, and use data for planning.

Healthy Families America Program Coordinator – Help Families Build Stronger Futures

Are you passionate about supporting children and families and helping programs deliver meaningful, high-quality services? We have an opportunity where you can make an impact at both the family and program level.

As the Healthy Families America (HFA) Program Coordinator at Home Healthcare, Hospice and Community Services, you’ll help coordinate our Healthy Families America home visiting program within the Family Resource Center. You’ll support program fidelity, accreditation readiness, quality improvement, staff development, data and reporting, and community engagement while also maintaining an assigned caseload of families.

This role is ideal for someone who brings a strengths-based, trauma-informed approach to working with families and enjoys building collaborative relationships with staff, community partners, and other stakeholders.

What You'll Do

• Coordinate the Healthy Families America program and support fidelity to HFA Best Practice Standards.
• Help develop and monitor program goals, objectives, performance outcomes, policies, and procedures.
• Support accreditation readiness, quality assurance, continuous quality improvement, case record reviews, and program audits.
• Assist with recruiting, hiring, training, supervision, coaching, and professional development of program staff.
• Promote reflective supervision practices and help monitor caseload assignments, staffing patterns, and workload distribution.
• Maintain an assigned caseload and conduct home visits according to HFA model requirements.
• Partner with families to develop individualized Family Goal Plans and support progress toward identified goals.
• Provide education and support related to parenting, child development, child safety, prenatal and maternal health, family wellness, and self-sufficiency.
• Complete required screenings and assessments and connect families with healthcare, mental health, housing, employment, education, and social service resources.
• Develop Family Resource Center activities that support HFA program goals.
• Oversee program data collection, accuracy, documentation, and reporting and use data to support planning and improvement.
• Build collaborative partnerships with healthcare providers, hospitals, schools, social service agencies, and community organizations.
• Represent HCS in local and state collaborative initiatives supporting children and families.

Requirements

Required

• Bachelor’s degree in Social Work, Psychology, Human Services, Public Health, Nursing, Early Childhood Education, or a related field.
• Five or more years of experience working with children and families.
• Prior experience with program coordination or working within a matrix program structure.
• Strong knowledge of child development and family systems.
• Understanding of trauma-informed and strengths-based practices.
• Knowledge of reflective supervision principles.
• Strong communication and relationship-building abilities.
• Ability to manage multiple priorities while meeting deadlines.
• Completion of all Healthy Families America required leadership and role-specific training within six months.
• Ability to maintain training requirements established by Healthy Families America and applicable funding agencies.
• Participation in ongoing professional development and continuing education activities.

Preferred

• Master’s degree in Social Work (MSW), Public Health (MPH), Counseling, Psychology, Nursing, or a related field.
• Experience with evidence-based home visiting, maternal and child health, child welfare, behavioral health, or family support services.
• Experience with accreditation, quality improvement, and grant-funded programs.
• Experience managing Healthy Families America, Nurse-Family Partnership, Parents as Teachers, or another evidence-based home visiting program.
• Experience leading accreditation or regulatory reviews.
