You will educate patients and families on post-acute options and assist with discharge planning. Responsibilities include completing assessments within 3 days of admission, coordinating care conferences, updating care plans, and supporting the healthcare

Formación

• Licensed social worker
• 1 year of supervised social work experience in a health care setting working directly with individuals

Responsabilidades

• Identify residents' social, emotional and psychological needs.
• Develop and implement care plans and discharge planning.
• Educate residents and families about post-acute options and issues of choice.
• Coordinate care with the interdisciplinary team for care conferences and related activities.
