Identify needs and connect clients with appropriate community resources, including referrals to CHWs.
Assist clients with navigating community services related to transportation, housing, food, childcare, and other social needs.
Conduct, support, and create workshops
Support outreach events.
Keep records and report to Coordinator on a regular basis.
Document progress of participant’s goals and administer pre/post assessments.
Collaborate with staff, fellow AmeriCorps members, and partner organizations to develop and support service projects in the primary areas of healthy futures.

Job description

The Healthcare Resource Specialist is a key member of the REACH Partnership within the Advanced Learning Department. This position is responsible for supporting comprehensive care management by facilitating annual Social Determinants of Health (SDOH) screenings, connecting clients to appropriate community resources, and collaborating with community partners and health worker teams. The Specialist will also deliver educational workshops in targeted census tracts within Danville City and Pittsylvania County, with a focus on chronic health conditions and identified population health needs. This is a nine-month service role requiring approximately 30 hours per week, with opportunities for additional training and continued service upon successful completion.
