AmeriCorps is seeking a Community Health Navigator in Richmond, Texas to serve as a critical link between clinics and community resources. This role focuses on supporting patients with social determinants of health and navigating healthcare systems effectively.
The position involves coordinating community health initiatives, providing digital navigation assistance, and supporting individuals in accessing vital resources and improving health outcomes. Ideal candidates will engage actively in outreach and public health programming.
The CHN for AccessHealth is a frontline public health worker who serves as a liaison and link between AccessHealth clinics, patients, community organizations, and health/social services. Community Health Navigators support individuals in addressing Social Determinants of Health (SDOH), navigating healthcare systems, accessing community resources, understanding healthcare information, utilizing digital healthcare tools, and engaging with programs and services that improve health outcomes and overall well‑being. In addition to traditional resource navigation activities, Community Health Navigators may participate in health literacy support, digital navigation assistance, patient engagement activities, community outreach initiatives, volunteer engagement efforts, and public health programming designed to improve healthcare access and address community needs.
