Job summary

Carease Health is seeking a Healthcare Regional Manager to oversee preventative care programs across a group of skilled nursing facilities. You will ensure service delivery, build relationships with DONs and Administrators, and maintain CMS-aligned regulatory compliance.

The role requires traveling to facilities, leading on-site teams, and coordinating monthly strategy meetings to drive performance and improvements in patient care.

• Experience in Skilled Nursing Facilities (SNFs) preferred.

Responsibilities

• Oversee Service Delivery: Ensure successful implementation and day-to-day operation of Carease's preventative care programs within assigned SNFs.
• Build Strong Relationships: Primary contact for facility leadership (DONs and Administrators) to ensure alignment with Carease's services.
• Ensure Regulatory Compliance: Oversee adherence to program protocols (RPM, CCM, CoCM) per CMS guidelines and company standards.
• Support On-Site Teams: Help integrate providers, care companions, and CNAs into facility workflow.
• Conduct Facility Visits: Travel regularly to monitor performance and reinforce best practices.
• Lead Monthly Strategy Meetings: Review metrics and align on goals and initiatives.
• Resolve Technical Issues: Coordinate and escalate any tech-related concerns to avoid service disruption.

As a Healthcare Regional Manager, you will be responsible for overseeing service delivery, building strong relationships with facility leadership, ensuring regulatory compliance, and supporting on-site teams.

Job Responsibilities:

Competitive Salary:

$80,000 - $90,000 ANNUALLY
