Job summary

The Samaritan Daytop Village is seeking a Quality Improvement Specialist to lead the organization’s quality activities and ensure adherence to Health Home policies. The role focuses on monitoring, implementing and evaluating program processes and clinical outcomes.

You will train new staff, oversee documentation, and generate monthly performance reports while coordinating with the billing team to resolve issues and ensure timely care delivery within the community.

Qualifications

• Associates Degree in Psychology, Human Services, Social Work, or related field.
• 3-5 years in Process Improvement and Continuous Quality Improvement, health home experience preferred.
• Proficiency in Microsoft Office and EHR systems.
• Strong interpersonal, communication, analytical and organizational skills.
• Experience working with diverse populations.

Responsibilities

• Implement and monitor the organization’s quality improvement program.
• Monitor and measure performance improvement activities and services.
• Provide monthly, quarterly and annual Performance Improvement reports and trend analyses.
• Coordinate quality improvement activities and ensure compliance with health home policies.
• Coordinate care across workers and document adherence.
• Train Outreach Staff and perform compliance reviews.
• Monitor Health Risk Assessments and Care Plans completion.
• Ensure daily entries are submitted.
• Collaborate with billing to resolve account and claim issues.
