You will perform routine rounds, manage chronic diseases proactively, and coordinate care across interdisciplinary teams with facility partners.

Qualifications

• Current NP or PA licensure in the state of practice.
• Previous professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.

• Strong collaboration across interdisciplinary teams.
