Job summary

Daybreak Independent Services, Inc. in New York is seeking a Medical Coordinator to ensure residences operate in accordance with agency policies and all regulatory requirements. You will manage calendars, coordinate appointments, and communicate with nurses and management to maintain seamless care.

You will handle medication changes, pharmacy coordination, and documentation, with a Monday–Friday eight-hour shift and compensation between $20.00 and $21.00 per hour.

Qualifications

• Bachelor's or 2 years experience in the field.
• OPWDD: 1 year.

Responsibilities

• Manage calendars and appointments for the team and ensure timely updates.
• Notify team if providers cancel or changes occur to appointments.
• Coordinate medication changes with the nurse and pharmacy.
• Track medication inventory and reorder as needed.
• Communicate staffing needs for 1:1 coverage.
• Obtain MD consults and ensure forms are completed properly.
• Follow seizure protocols and renew scripts as required.
• Review medical consults and documents before sharing with the team.
• Schedule all appointments within the allotted timeframes and avoid overlaps.
• Transport individuals to/from medical appointments.
• Ensure all lab/test results are filed and available in the binder.
• Request lab/test results timely via patient portal.
• Schedule and complete specialty tests within the target windows.
• Prepare and submit pharmacy reviews to MDs in a timely fashion.

Education
